Small Changes Can Raise Important Questions.
A fall or a gradual change in day-to-day ability may lead to questions about safety, support and what should happen next.
Families may notice changes in how an older person walks, manages everyday activities, leaves the house or copes with routines that were previously manageable.
AgeWell Nursing provides general nursing guidance to help families organise these observations and prepare questions for the appropriate healthcare professionals.
Changes Worth Talking About.
Families may become concerned when an older person’s mobility, confidence or ability to manage their usual routine begins to change.
01 Increasing difficulty with everyday activities.
02 Reduced confidence when walking or leaving the home.
03 Repeated falls or near-falls.
04 Changes in appetite, strength or energy.
05 Needing more help with personal care.
06 A general change in independence that is difficult to explain.
Talk Through Falls, Changes And Concerns.
A fall, near-fall or change in confidence can raise questions about an older person's safety, mobility and independence.
Your consultation provides dedicated time to talk through what has been happening, organise your concerns and consider practical areas that may need further attention or discussion.
AgeWell Nursing provides education and nursing guidance. It does not diagnose the cause of falls, provide a physical assessment or replace assessment by an appropriate healthcare professional.

Education and guidance

Falls can have many different causes.
AgeWell Nursing can provide education and guidance about falls and help you organise the concerns and changes you have noticed. An online consultation cannot determine why a fall has occurred or replace assessment by an appropriate healthcare professional. If an older person is injured, suddenly unwell or requires urgent medical attention, seek appropriate medical care.
You do not have to navigate this alone.
Bring your questions, concerns and what has changed. Caroline will help you organise the situation and understand what to do next.
