Changing Needs Can Bring Difficult Decisions.
When an older person’s needs change, families may need to consider a range of options. These might include support at home, community services, additional family support or residential care.
These conversations can feel emotional and complex, particularly when family members are unsure what level of support is needed or whether current arrangements are still working.
AgeWell Nursing provides an independent nursing perspective to help families organise their questions and think through the issues they may need to explore with the appropriate services and professionals.
From uncertainty to a clearer next step.
Choose the consultation that best matches your concern, book a suitable time and meet privately with Caroline by Zoom or Microsoft Teams.

Available across Ireland
Consultations are available online to older people, families and those supporting an older person throughout Ireland.
Meet privately online
Your consultation takes place privately by Zoom or Microsoft Teams, so you can join from wherever suits you.
Who can book
An older person can book for themselves, or a family member or someone supporting an older person can arrange a consultation.
No formal preparation
You do not need to prepare documents or have everything figured out. Simply bring the questions and concerns you would like to talk through.
Guidance and education
The consultation provides nursing guidance and education. It is not a physical assessment, diagnosis or substitute for an individual care plan from your treating healthcare team.
You do not need to have made a decision about residential care.
You may be considering additional support at home, exploring community services, beginning to think about residential care, or simply trying to understand the options available. The consultation is a space to organise your questions before deciding what may need further discussion.
You do not have to navigate this alone.
Bring your questions, concerns and what has changed. Caroline will help you organise the situation and understand what to do next.
