When More Support At Home May Be Needed.
An older person’s needs can change gradually or following an illness, hospital admission, fall or other change in circumstances. Families may begin to notice that everyday activities are becoming more difficult or that existing arrangements are no longer working as well as they once did.
It can then be difficult to know what type of help to explore, which questions to ask or who may need to be involved.
AgeWell Nursing provides an independent nursing perspective to help families organise these questions and prepare for discussions with the appropriate healthcare, community and support services.
What Support Does Your Family Need To Explore?
Every situation is different. A consultation can help you identify the questions that matter most for the older person and family.
01 Is the older person managing everyday activities safely?
02 What areas of daily life are becoming more difficult?
03 What types of home support may need to be explored?
04 Are current family arrangements still manageable?
05 Which healthcare or community professionals may need to be involved?
06 How can the older person's preferences remain central to discussions?
Make Sense Of Home Support Options.
When an older person begins to need more help at home, it can be difficult to know what support may be appropriate or which questions to ask.
Your consultation provides dedicated time to talk through the older person's current needs, existing family arrangements and the areas where additional support may need to be explored.
AgeWell Nursing provides independent nursing guidance and education. We do not provide, arrange or assess eligibility for home support services.
