Older People.
Many older people want to remain living at home and maintain their independence for as long as possible. Independence Can Mean Different Things To Different People.
For one person, independence may mean continuing to prepare meals, manage everyday routines or get out into the community. For another, it may mean remaining involved in decisions while receiving more help with particular activities.
Needs can also change over time. Families may begin noticing changes in mobility, confidence, routines or the amount of support an older person needs.
AgeWell Nursing provides a calm space to talk through these changes, organise concerns and consider what questions may need to be raised with the relevant healthcare or support professionals.
When Everyday Life Starts To Feel Different.
A change does not automatically mean that someone can no longer live independently. It may, however, be a useful time to consider what has changed and whether further advice or support should be explored.
01 Everyday tasks are becoming more difficult or taking longer.
02 Mobility, balance or confidence around the home has changed.
03 The older person is becoming less confident about going out.
04 Family members are providing increasing amounts of support.
05 A recent illness, fall or hospital stay has affected confidence.
06 Existing arrangements no longer feel as manageable as before.
A Conversation Built Around Your Situation.
You do not need to arrive with a complete list of solutions. Start with the changes, questions or concerns that matter to you.
Your consultation provides dedicated time to talk through what has changed, consider what independence means to the older person and organise the questions you may want to explore next.
