Caring For An Older Relative Can Become Complicated.
Supporting an older parent or relative can become complicated when their health, mobility, independence or daily needs begin to change.
Families may be unsure whether they are worrying too much, not doing enough or asking the right questions.
AgeWell Nursing offers independent online nursing guidance to help families organise what they are seeing and prepare for appropriate next steps.
You May Simply Know That Something Has Changed.
Families often seek guidance when they can see that an older person’s circumstances are changing but are unsure what should happen next.
01 Noticing that an older person is struggling more at home.
02 Trying to understand what support may be available.
03 Preparing for a discussion with a GP or hospital team.
04 Finding it difficult to coordinate different family views.
05 Considering home support, community services or residential care.
06 Unsure which questions to raise with healthcare professionals.
Make Sense Of What Is Changing And What Comes Next.
When an older person's needs begin to change, families can quickly find themselves trying to understand several concerns, services and decisions at the same time.
A private consultation gives you dedicated time to talk through what is happening, organise the issues that matter most and identify the questions that may need to be raised with the appropriate people or services.
You do not need to know exactly what type of support is needed before booking. Family Care Navigation is designed to help you make sense of the situation and consider what may need to be explored next.
Start With The Concerns You Have Right Now.
Talk through what is happening
Bring the changes, concerns and questions your family is currently trying to understand.
Organise the concerns
Work through the different issues and identify which areas may need further attention or discussion.
Prepare for next conversations
Identify clearer questions to discuss with family, healthcare professionals or relevant support services.
A lifetime of experience, offered with warmth and honesty.
